No appointment needed — just walk in during our hours. When we open, we'll also offer online check-in so you can add yourself to the queue before you arrive and reduce your wait time.
We're open Monday through Friday 3:00 PM – 11:00 PM, and Saturday and Sunday 10:00 AM – 6:00 PM (same hours both days). Holiday hours will be posted in advance. Our evening and weekend hours are designed specifically for patients who can't take time off work.
We're on the 4th floor of DM Plaza at 167-14 Hillside Ave, Jamaica, Queens, NY 11432. The building has elevator access. Street parking is available nearby and we're accessible by public transit. A full directions guide will be available at launch.
Most patients are seen within 30–45 minutes of arrival. When our online check-in system launches, you'll be able to join the queue before you leave home, which significantly reduces in-office wait time.
Bring a valid photo ID, your insurance card (if you have one), a list of any current medications, and a form of payment for any copays or self-pay fees. If you're a minor, a parent or guardian must accompany you.
We accept Medicare, Medicaid, and all major commercial plans including Aetna, UnitedHealthcare, Blue Cross Blue Shield, and Cigna. See our full insurance page for the complete list. Coverage depends on your specific plan — we recommend calling your insurer to confirm urgent care is covered before your visit.
We offer transparent self-pay rates. A standard visit starts at $125 — significantly less than an ER visit. Rapid lab tests are $45 and physicals are $95. You'll always know your cost upfront. No surprises.
Yes. We bill your insurance directly and you're only responsible for your copay or any applicable deductible at the time of your visit. If there are any balance billing questions, our team will work with you to resolve them.
If you're experiencing chest pain, difficulty breathing, signs of stroke (face drooping, arm weakness, speech difficulty), severe bleeding, loss of consciousness, or any life-threatening emergency — call 911 or go directly to the ER. Urgent care is for conditions that need prompt attention but are not life-threatening.
Yes, we treat patients of all ages including children. A parent or legal guardian must be present and provide consent for any minor's treatment.
Yes. Our providers can prescribe non-controlled medications when clinically appropriate. We can send prescriptions electronically to any pharmacy. We do not prescribe controlled substances.
Yes. Visit summaries and records are available upon request. We can also send records to your primary care physician or a specialist with your written authorization.
Yes — our team speaks English, Bengali (বাংলা), Urdu, and Tagalog. We want every patient to feel understood and comfortable, and we know that language is a big part of that. No interpreter needed.
Yes. Top Level Urgent Care was founded by Dr. Mohammed M. Rahman, who has practiced medicine in Queens for 25 years — 10 years in Woodhaven and nearly 15 years in Jamaica through Saireen Medical Group at 88-34 161st St. Top Level is an extension of that practice — same commitment to the community, now with evening and weekend urgent care access. Patients of Saireen Medical Group are always welcome.
DM Plaza is located at 167-14 Hillside Ave in Jamaica, Queens. Top Level Urgent Care is on the 4th floor with elevator access. The building also houses a restaurant, grocery store, and event space — all part of a community-focused development serving Jamaica and the surrounding neighborhoods.
